Medical Billing Software vs. a Billing Service: Who Does the Work?

Start with the responsibility you need filled. Software provides tools; a billing service performs the work assigned in its agreement. A practice may need better tools, staff to operate its existing system, or both.

Compare the tool with the person responsible for the queue

A software demonstration can show claim submission, payment posting and a reporting dashboard. Ask who uses those functions every day. Which person reviews a rejected claim, obtains missing information, corrects it and confirms the next response? A feature list does not assign that responsibility.

A billing service proposal should name the work it covers and the exceptions it returns to the practice. Synergy can assess hands-on work in your existing billing environment, subject to the agreed assignment and authorized access. We do not assume that every platform feature is available in your installation or that every billing task is included.

Test ERA posting and EFT reconciliation as separate steps

An electronic remittance advice, or ERA, explains a payer's adjudication and adjustments. An electronic funds transfer, or EFT, moves the payment. CMS describes reassociation as matching the payment with its corresponding remittance. Receiving an ERA is therefore different from confirming that its payment matches the deposit.

For a hypothetical demonstration, ask the vendor to show a normal remittance, a payment that does not match, and a provider-level adjustment. Then ask the proposed billing team who investigates each exception, where the action is recorded and who approves an adjustment. ERA information can support automated posting, but the practice still needs a defined exception process. Do not infer a platform's matching or posting behavior without seeing its configuration.

Separate claim rejections, denials and follow-up ownership

Use an example work list in the buying conversation. Include a rejected submission, an adjudicated denial, an unpaid claim and a request for documentation. For each item, identify the person responsible for the next action, the access required and the escalation contact. Use synthetic examples during an initial demonstration.

Ask how you will see completed actions and unresolved work. A dashboard can report an outstanding balance without showing whether someone contacted the payer or what response is needed from the office. Agree what the service will record and when the practice must respond.

Decide whether to keep the system, change it or add operational help

If your staff can complete the work but the software lacks a required function, a software review may be the useful next step. If the tools already exist and queues are not being worked, discuss staffing or an assigned billing service. If both are changing, plan the responsibilities during the transition as well as after go-live.

An independent physician using a hospital-hosted system should also identify who can authorize third-party access and which functions that access includes. A billing contract does not grant hospital permissions. Confirm the exact practice, host and workflow before committing to a start date.

Compare the complete proposed scope before comparing fees

List software licensing, clearinghouse or transaction charges, operational staffing and the work excluded from the service proposal. Ask whether patient statements, older balances, secondary claims and payment reconciliation are included or separately scoped. Confirm who owns the data and how open work is handed back when the assignment ends.

Synergy's first conversation can identify the software you use, the queue that needs attention and the responsibilities your office wants to retain. We then discuss access and a defined proposal. This guide does not rank products or promise that outsourcing is the right choice for every practice.

See how assigned work differs from the software

Our anonymous Medisoft case describes secondary-claim follow-up while the practice retained initial billing. Our AdvancedMD case compares quarterly payment reports and explains their reporting limits. These examples explain their reported scope and measurement limits; neither establishes the same result for another practice.

For the financial comparison, use our in-house versus outsourced billing calculator. For a proposal, review billing service pricing and contract questions.

What to discuss first

  • Your existing EHR, billing platform and clearinghouse, including who grants access.
  • The tasks you need someone to perform, rather than the features you want demonstrated.
  • The person responsible for ERA exceptions, deposit matching and adjustment approval.
  • The office contact for coding, documentation and patient-account decisions.
  • The reporting, excluded work, fees and handoff terms in each written proposal.

Common questions

Do we have to replace our software to hire a billing service?

Discuss your current environment first. Synergy can assess an assignment in an existing system, but the available workflow, permissions and scope must be confirmed for that practice. A software migration is a separate decision.

Does automatic ERA posting mean nobody needs to reconcile payments?

No. Ask how the actual installation matches a remittance to a payment, handles mismatches and records approved adjustments. Assign responsibility for exceptions even when routine items can post automatically.

Can you compare every vendor's automation features?

This guide provides questions for your vendor demonstration and billing proposal. Features depend on the product, configuration and access. We verify the workflow for a proposed assignment rather than publish an unverified ranking of platforms.
